Registration Terms and Conditions
The following Registration Terms & Conditions (the “Terms & Conditions”) apply to all Prevention Education & Referral Services, Inc. programs and services. Prior to your registration, you must acknowledge and accept the Terms & Conditions contained herein. Should you not wish to accept the Terms & Conditions you should not register.

Payment
Payment for PERS programs is required in full at the time of registration. Payments can be made by debit card, credit card or by money order, and must be in the registrants name. Any unauthorized use of that results in a disputed payment(s) will result in an administrative fee of $55 per dispute and reported and prosecuted accordingly. All payments are non-refundable and non-transferrable.
Program Rescheduling
Registrants who are unable to attend their scheduled PERS program, are required to provide at least 48 hours advance notice in order to reschedule without financial penalty. Registrants will incur a $35 rescheduling fee if rescheduling the missed session within 5 days of the missed program date if rescheduling to a date within the following 8 weeks of the initially scheduled program date.
Program Conduct and Safety
PERS is committed to providing a safe, productive, and welcoming environment to all participants, including staff, at PERS programs and courses. PERS has no tolerance for discrimination, harassment, or bullying in any form at PERS-related events. Participants are expected to adhere to these principles and respect the rights of others.
